Animal Care and Housing Declaration Form for 4-H Animal Projects

This declaration form is now electronic through Google Forms. Please follow this link to complete your declaration: https://tinyurl.com/4-HCHDeclaration23.

*NEW FOR 2023*: If your project animals are housed at a property other than your home residence (whether an ownership or non-ownership project), please send this separate link: https://tinyurl.com/4-HCHDeclarationFarmer23 to the farmer/caretaker of the property where your animals are kept. Completing this separate form will serve as their acknowledgement of your project plan and has the same deadline as your main form (see below).
